Let’s Make It Official Inc. and the City of Tampa Park and Recreation Dept./Springhill Park Community Center the receipt of which is hereby acknowledged, I hereby discharge and release Let’s Make It Official, Inc. and all sponsors of the league as well as their employees, officers, volunteers, and agents, of and from any and all liabilities, claims, demands, and cause of action, whatsoever arising out of or related to any injury (including death) which may be sustained by me as a result of my participation in the League. I further acknowledge that I am duly aware of the risk and hazards, which may be associated with my participation on the basketball league and hereby elect voluntarily to participate and to reassume all such risks and hazards
